FREE SERVICE FOR CLIENT
Our services are free to the client in search for care options.
A referral fee is paid to Elder Placements after we successfully match the client with the appropriate care option.

We understand that this is a difficult decision to make, therefore we want to help you make an informed decision. In our 24 years of consulting, referral and placement experience we have put great value in making this experience as personal and smooth as possible.
Our goal is to ease the anxiety and stress that this process can bring and work along our clients’ families in order to make the transition as easy as possible. Therefore, we approach this process in three easy steps.
1. Meet & Evaluate
Meet personally with our potential client and their family and begin an intake process, an initial client evaluation of care needs followed by an in-depth Care Needs Assessment.
We will connect the client with a qualified Assessor. This is a state requirement, and it also provides the tool that directs the
patient’s care, while it provides the Care facility with the care details needed
to determine the appropriate care cost.
2. Tour Best Options
Select 3 best options and tour with the client and/or family members.
We start our work to find the best options that will specifically meet our
client’s needs.
We pre-screen all care providers we work with. Once we have fine-tuned your options, we will accompany you on tours of the best
matched facilities.
3. Facilitate Transition
We will help with the steps of transition and facilitate the admission into the
new facility.
Our goal is you're complete satisfaction.
We will do whatever is necessary to provide the best transition, as well as follow up with our clients to ensure that care needs are being met by the facility and their staff.
